Through our Community Care Hub (CCH), we connect healthcare providers, social services, and community partners to make sure individuals and families in Southwest Colorado don’t fall through the cracks.

Now, we’re looking for a Senior Program Director to guide this important work.

This isn’t just a leadership role—it’s an opportunity to shape how our region supports health and well-being. The Community Care Hub serves as the bridge between people and the resources they need most, and as Program Director, you’ll help build those connections, foster trust, and drive real impact.

Every day will look a little different. Some days you’ll be working alongside your team of care and outreach coordinators, supporting them as they walk with families through tough challenges. Other days, you’ll be at the table with hospital leaders, local government, or nonprofit partners, creating systems of care that put people first. You’ll keep a close eye on budgets and data, not just to check boxes, but to ensure we’re continually improving, innovating, and proving the value of what we do.

You’ll also be the connector—the one who sees the bigger picture, who can align diverse partners toward a shared vision of healthier, stronger communities.

Job Title:
Senior Program Director – Community Care Hub
FLSA: Salary/Exempt Full-Time
Reports To: President/CEO

Location:
Hybrid. Must be located within the region of Southwest Colorado to effectively collaborate with community partners.

Position Overview:

The Program Director for the Community Care Hub will lead the development, implementation, and continuous improvement of community-based programs designed to enhance the health and well-being of individuals and families in the local community. This leadership role requires a strategic thinker who can oversee program operations, manage resources effectively, and foster partnerships with community organizations, healthcare providers, and other stakeholders. The Program Director will ensure the delivery of high-quality, culturally competent services while promoting the mission and values of the Community Care Hub.

Key Responsibilities:

1.     Program Leadership & Management:

o    Lead and manage all aspects of the Community Care Hub programs, including social support, education, and wellness initiatives.
o    Ensure programs are aligned with community needs, organizational goals, and best practices.
o    Develop, implement, and evaluate new programs and services that address gaps in community care.
o    Oversee the Integrated Care Coordination Team through the Regional Accountable Entity program.
o    Oversee day-to-day operations, including staffing, scheduling, and service delivery.
o    Participate in agency-level decisions that impact the Community Care Hub program or personnel.

2.     Team Management & Development:

o    Supervise and support program staff, including care coordinators and outreach coordinators.
o    Provide ongoing professional development opportunities, performance evaluations, and feedback for staff.
o    Foster a positive, collaborative, and culturally competent work environment.
o    Ensure staff are trained on program protocols, policies, and community care standards.
 
3.     Community Engagement & Partnership Building:

o    Build and maintain strong relationships with community partners, including healthcare organizations, social service agencies, schools, and local government.
o    Act as the main point of contact between the Community Care Hub and external partners.
o    Advocate for the needs of the community and promote the organization’s mission through outreach and community events.
o    Collaborate with community leaders to identify emerging needs and create responsive programs.
o    Facilitate Community Care Hub Advisory Council and other advisory committees.
 
4.     Resource & Budget Management:

o    Develop and manage program budgets, ensuring the efficient use of resources and adherence to financial guidelines.
o    Identify and pursue funding opportunities, including grants, donations, and other revenue streams.
o    Ensure compliance with all financial reporting, auditing, and regulatory requirements.

5.     IT System Oversight

o    Ensure the hub’s hardware, software, and network systems are secure, reliable, and scalable to support service delivery.
o    Implement cybersecurity protocols and risk frameworks to protect sensitive client and organizational data in compliance with regulatory standards.
o    Provide or coordinate technical support for staff and network partners, including troubleshooting and system maintenance.
o    Manage platforms that facilitate secure data sharing between healthcare payors, service providers, and the hub itself.
o    Collect, analyze, and report data on service utilization, outcomes, and performance to inform strategic decisions and continuous improvement efforts.
o    Use data systems to measure the effectiveness of services and demonstrate value to stakeholders and funders.
o    Collaborate with senior managers to identify barriers and develop systems, policies, and procedures that improve operational efficiency.
o    Train staff on new or updated IT systems, ensuring consistent use and compliance with protocols.
o    Evaluate and manage relationships with IT vendors, including software providers and consultants.

6.     Data Management & Reporting:

o    Collect, analyze, and report on program performance, including service outcomes, client satisfaction, and impact metrics.
o    Use data to make informed decisions and to refine program strategies.
o    Prepare and present reports for internal leadership, funding agencies, and community stakeholders.

7.     Quality Improvement & Compliance:

o    Monitor and evaluate the effectiveness of programs and services, making adjustments as necessary.
o    Ensure all programs meet local, state, and federal regulatory requirements, as well as industry standards.
o    Lead efforts to implement quality improvement initiatives and maintain high standards of care.

8.     Crisis Management & Problem Solving:

o    Provide guidance and support for handling crisis situations, ensuring the safety and well-being of clients and staff.
o    Address issues and challenges as they arise, implementing solutions to mitigate risks and maintain program integrity.

9.     Other reasonable tasks requested or delegated by supervisor or agency leadership. Qualifications:

·        Education & Experience:

o    Bachelor's degree in social work, public health, nonprofit management, or a related field required (Master’s degree preferred). Degree requirement may be waived for candidates with significant (10+ years) applicable experience.
o    Minimum of 5 years of experience in program management, with at least 3 years in a leadership role, preferably in a healthcare or community-based setting.
o    Experience in managing interdisciplinary teams and complex programs.
o    Knowledge of community health systems, social services, and local resources.

Working Conditions:

Physical Demands of the Job: These essential tasks may be performed with or without accommodation or assistive technology. Employees in this position may spend significant hours on the computer writing and entering information into a variety of information systems. Employees in this position use laptop computers and may also have documents and files that they will need to transport within and outside the office. The employee must be able to read text on the computer and paper documents. Communicating with others via email, letters and telephone is required. This position frequently requires the employee to attend meetings, training, and other community activities within Southwest Colorado. This position requires the employee to perform visits in client homes and make visual inspections of environmental conditions.
