Compensation: $40.00-$58.67 (DOE).
Sign-on Bonus: $2,000, paid after 90 days of employment.
Location: South Bend and Sunriver.
Schedule: Full-time, Monday- Friday, 8 am-5 pm.
Travel: Local/regional travel by personal vehicle during the working day. Gas card and mileage reimbursement are provided.
What You Will Do:
- Complete an initial comprehensive and accurate assessment of the patient and family to determine home health needs.
- Develop an individualized care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Include the patient and the family in the planning process.
- Initiate appropriate preventive and rehabilitative nursing procedures and treatments. Administer medications and treatments as prescribed by the physician.
- Ensure that arrangements for equipment and other necessary items and services are available.
- Provide health care instructions and education to the patient and or family as appropriate per the assessment plan and the patients’ caregivers’ ability to learn. Adapt teaching styles to meet patient’s needs and family’s needs.
- Identify discharge-planning needs as part of the initial care plan development and implement prior to the patient's discharge.
- Participate in weeknight and weekend on-call duties as defined by the on-call policy.
Required Skills & Qualifications:
- Valid Oregon RN License.
- Ability to pass a background test, drug screen, and TB test.
- Graduate of an accredited nursing school.
- One to two years of recent medical care experience in an institutional setting is preferred. Ideal candidates will possess 1-2 years of home health or hospice experience.
- Current automobile insurance is required. A valid Oregon driver’s license and reliable transportation.
- Valid CPR Certification.
- Strong communication and interpersonal skills.
- Ability to work independently and collaboratively within an interdisciplinary team.
