ABOUT THE ROLE
As the Case Manager, you will be an essential team member responsible for ensuring individuals and families enrolled in the Family Stability Program meet the program goals and objectives. Case management is a mixture of project management and administrative functions, all in support of the success of Elev8 Baltimore. Reporting to the Wholeness Senior Director, you will provide full program support to a case load of up to 35 individuals/families.
 
You will be instrumental in supporting three (3) core areas of work in the Mondawmin/Liberty Heights neighborhoods:
  1. Identification of stable housing for residents of the target area and prevention of homelessness
  2. Ensuring family stability
  3. And increasing self-sufficiency.
You will interface with Elev8 staff, leading community agencies and resources, and relevant city agencies aligned with the objectives of this initiative. 
 
Your Tasks and Responsibilities:
  • Promote, recruit, and enroll individuals in the program in the Mondawmin/Liberty Heights neighborhoods
  • Assess program participants to identify appropriate supports (financial, transportation, court involvement, child support and/or other related assessments) 
  • Maintain detailed case notes in Apricot 360 data collection software
  • Utilize best practices methods from the area of housing identification
  • Provide financial, workforce and continuing education coaching support
  • Provide retention and follow up services and supports to program participants 
  • Cross promote Elev8 Baltimore programs (e.g. South Baltimore Adult High School, Extended Learning (aka Out of School Time), Leadership Development, etc.) 
  • Participate in school-level team meetings aligned with the strategy
  • Attend required meetings for program
  • Draft and edit memos, correspondence and outreach communications
  • Perform other job-related duties, as assigned
Qualifications
  • Passionate commitment to improving communities of color, urban public schools and to the mission Elev8 Baltimore 
  • Bachelor’s degree preferred
  • Professional, with two to four (2-4) years of experience providing case management within housing, education, youth development, workforce development and/or community development programs
  • Excellent project management, problem-solving and analytical skills 
  • Ability to develop and maintain effective and positive working relationships with a broad range of stakeholders including parents, educators, community residents, elected officials, funders, and the business community
  • Ability to learn quickly and succeed in the rapidly changing environment of a start-up project
  • Ability to produce high-quality work and great volume in a mission-driven, results-oriented culture, and to respond appropriately in pressure situations with a calm and even temperament
  • Dependability, initiative, and ability to follow-through on assignments
  • Proficient in the use of Zoom, Microsoft Office software, including PowerPoint software to prepare presentations for the Elev8 Baltimore team
  • Proven organizational skills with the ability to handle multiple tasks, track details and plan and act proactively 
  • Strong verbal and written communication skills 
  • Strong interpersonal skills and ability to work independently and as a team
  • Ability to function with minimal supervision and manage multiple tasks
  • Must possess excellent organization and time management skills.
  • Must possess a strong work ethic, positive team attitude, excellent problem solving and interpersonal skills
  • Dependability, initiative, and ability to follow through on assignments.

Work Environment
This position will work out of the main Elev8 Baltimore office located at 210 W 28th Street, Baltimore, MD 21211 and maintain regular office hours in the schools to support ease of access for community.

Core Work Hours are 8:30 am – 5:30 pm. The Initiative hours can begin as early as 7:00 am and run as late as 7:00 pm. Pre-planned activities can include weekend and evening hours. There may be occasional emergency occurrences where extended hours to support clients are necessary.

Travel:
  •  Local travel may be needed to/from our office in Remington to/from a school site or other location in Baltimore City.

Compensation
  • Hourly rate of $23.08. A comprehensive benefits package is provided.
