Your Impact:

Empower families on their journey to self-sufficiency by providing critical support as an SFC Case Manager all while working closely with the Data Clerk. Be a hero for families in need by ensuring they receive uninterrupted services through efficient home visits and document processing. Proactively visit clients in their homes, reviewing essential documents, and gathering necessary signatures to keep their services running smoothly.

Requirements:

  • Experience and Education: Minimum of an Associate's Degree in Human Services or similar field with 2 years of relevant experience; or a Bachelor's Degree in any field and 3 years of relevant experience.
Bonus points:

  • Prior experience with Medicaid waivers.
  • Training in Person-Centered Thinking/Planning.
