Moms In Motion has an immediate opening for a Case Manager.

Work from your home office. Must live in Virginia.

The Case Manager  is responsible for supporting the team and the families we serve by reviewing assessments, plan of cares, updated outcomes, supervision requests and justifications for Medicaid Waiver recipients. 

Skills and Qualifications include: 
  • Requires a documented combination of skills/relevant work experience. Person-Centered Thinking/Planning training is a plus. 
  • Knowledge of Virginia Medicaid Waivers a plus
  • Highly organized
  • Work remotely with electronic devices and associated tools and equipment required to complete day-to-day tasks. 
  • Maintains experience in a variety of applications and programs; not limited to but including Office, Google, & Adobe. 
  • Must maintain a steady and reliable internet connection. 
  • Have good written and verbal communication skills. 
  • Submit to a criminal background check. 
  • Two references are required.
