Job Summary: Based in the Douglas County school district, the School Based Health CCMA will act as a liaison between the school district and the Evergreen Family Medicine clinic, building a bridge to give access to healthcare to all students.
The School Based Health CCMA will collaborate with an EFM team (as well as with the Mercy Foundation, Cow Creek, and HKOP) to develop, plan, and support the implementation of the school-based health programs. The School Based Health CCMA also works closely with school and EFM administration to coordinate the school-based program activities. The School Based Health CCMA approaches his/her tasks in a team-based care fashion that supports patients and their families.
This position will be held by EFM clinical staff who may be required to prioritize patient care in the clinic setting when needed and when school is not in session.
Responsibilities and Duties:
• Works in conjunction with Medical Providers, clinical staff, and all other staff to ensure that patients obtain the highest standard of integrated healthcare
• Acts as a patient advocate for our vulnerable populations by ensuring that patients are fully informed and comfortable with their visit to EFM
• Follows procedures for the electronic health record system
• Provides support for quality control of school-based health equipment
• Oversees program fluidity within schools in the program
• Works with EFM immunization staff to organize vaccination events
• Provides guidance to school faculty regarding student medical inquiries
• Stays up to date on all pediatric and vaccine requirements, state, and federal regulations
• Maintains continuity among clinics, departments, and local work teams by documenting and communicating actions, irregularities, and continuing needs
• Maintains professional standards; reviewing professional publications; establishing personal networks
• Understands e-learning techniques and education
• Assist the management teams at the sites for pediatric and immunization training needs
• Involved with advertising and promotion of EFM School Based Health Programs
• Obtaining medical consent from parent of student and maintains good communication with parents throughout process
• Educates students and parents on appropriate medication administration dependent on provider prescription and medication (inhaler, insulin, etc.)
• Other duties as assigned
Qualifications and Skills:
• Certified Clinical Medical Assistant or Higher
• Excellent organizational and interpersonal skills
• Strong oral/written communication skills
• Effective time management skills
• Self-starter and an independent learner
• Analytical and problem-solving skills
• Must work well with the other patient care team members
• Has understanding of continual process improvement concept
• Ability to work independently and as a team member
• Proficiency with Microsoft Excel and comfortable working within a consolidation software system
• Must work well with the administration team
• Must work well with children
• Must have a valid drivers license.
• Experience working with a vaccine program and/or pediatric program preferred
• Experience with program management preferred
Physical requirements:
• Prolonged periods sitting at a desk and working on a computer.
• The employee is frequently required to walk; use hands and fingers, handle, or feel; and reach forward with hands and arms.
• The employee is occasionally required to sit and stoop, kneel, or crouch.
• May be exposed to patients with infectious diseases.
• Must be able to withstand physical and emotional stress.
• Requires long hours of standing and moving.
• Must be able to lift up to 15 - 25 pounds at times.

Compensation: $23.33 - $29.42 per hour
